As a Clinical Manager at One Care DC INC, you will play a pivotal role in overseeing and coordinating the holistic care of our clients. This position requires you to ensure that all clinical services meet the highest standards, while providing supervision and support to a team of Community Support Workers (CSWs). You will also collaborate with a multidisciplinary team of professionals, including psychiatrists, primary care physicians, and social workers, to ensure the best outcomes for our clients. Your leadership will be essential in maintaining quality care and fostering a collaborative environment for both staff and clients.Key Responsibilities:Supervision of Client Care: Oversee and coordinate the comprehensive care of clients, ensuring that treatment plans are executed as outlined, and services are delivered in accordance with the client’s Plan of Care.
CSW Supervision: Provide direct supervision and guidance to Community Support Workers (CSWs), ensuring they are following the treatment protocols and delivering quality care.
Collaboration with Providers: Coordinate care and collaborate with psychiatrists, primary care physicians, social workers, and other healthcare providers to ensure a holistic approach to client care.
Plan of Care Management: Ensure that client Plans of Care are followed, adjusted as necessary, and are regularly reviewed for efficacy.
Physiatrist and Therapy Services Coordination: Monitor that physiatrist services and therapy sessions are delivered according to the client’s treatment plan.
Documentation and Notes Review: Review and ensure the accuracy of client notes, progress reports, and treatment documentation. Maintain compliance with all relevant standards and regulations.
CSW Supervision: Supervise and evaluate Community Support Workers (CSWs), ensuring that they are adhering to care plans and organizational protocols.
DA Updates and Review: Regularly review and update Diagnostic Assessments (DAs) to ensure accurate and current information is reflected in the client’s record.
DLA-20 Administration: Complete and review DLA-20 (Daily Living Activities) assessments for clients, ensuring accuracy and thoroughness in evaluation.
Treatment Plan Approval: Sign and approve client treatment plans, ensuring that all interventions are appropriate and client-focused.
Client Discharges: Oversee and manage client discharges, ensuring a smooth transition and that all discharge planning is executed appropriately.
Face-to-Face Encounters: Conduct regular face-to-face encounters with both clients and CSWs to monitor progress, address concerns, and provide ongoing support and guidance.
